We Need Your Student Health Forms!
IMPORTANT: Currently, we are missing many of the required health forms for our students!
Please check the list here to see if we still need your form.

Don't worry; we've made it easy for you to submit them! You can upload the forms directly in the Parent Portal, send them to Chandler via email, or simply send the paper version with your child. Our priority is to ensure we have the necessary health information for each student to provide a safe and supportive learning environment.
If we do not receive your child's health forms, we will have to charge for the required Vision and Health screenings. The charges are as follows:
Vision Screening: $30
Vision and Hearing Screenings: $45
Vision, Hearing, and Scoliosis Screenings (Scoliosis screening is only required for 5th & 7th graders): $55
We understand that some parents may have already submitted their forms, but we kindly request that you double-check and resubmit if necessary. Your cooperation is greatly appreciated in helping us maintain a healthy and safe school environment.
